How we verify
Hospice Stars is compiled from public records, not from who paid to be listed.
Sources
- CMS Hospice General Information — Every Medicare-certified hospice: name, address, phone, ownership and certification date.
- CMS Hospice Provider Data — Quality measures (Hospice Care Index, comprehensive assessment, visits in the last days of life), daily census, care settings and diagnoses.
- CMS CAHPS Hospice Survey — Family caregiver survey results and the family survey star rating for each hospice.
We load three CMS files published for Medicare Care Compare and join them on each hospice's CMS certification number: general information, provider data (quality measures, census and care settings) and the CAHPS Hospice Survey. Every Medicare-certified hospice in the file is listed.
Family survey figures are the share of caregivers giving the most positive answer (for example “always” communicated well, or “definitely” would recommend). The star rating is CMS's CAHPS Hospice summary star rating. Quality measures are shown as CMS reports them, with their reporting periods. We do not adjust or combine CMS's numbers.
Categories such as “serves nursing home residents” come from CMS's share of care days by location (5% or more) and share of patients by diagnosis (30% or more for dementia). They describe who a hospice served, not a certification.
Order within a city: highest family survey star rating first, then the share of families who would definitely recommend, then the Hospice Care Index. Hospices without scores are listed after rated ones, alphabetically. Featured hospices appear in a separate, labelled box and never change that order.
What a badge means
Certified: the hospice holds an active CMS certification number and appears in the current CMS Hospice General Information file. We only publish positive matches. A listing without a badge is not an accusation — most gaps are name variations between registries.
Freshness
Data was last refreshed on 2026-09-30. CMS refreshes hospice data quarterly; we re-pull it weekly.
